HOW TO CHECK-OUT HEALTH EDUCATION RESOURCES

A.  BASIC GUIDELINES

1. Only members of the TFCNN will be able to check out resources. 

2. Items may be checked out for a maximum of 5 days.

3. Make sure all the resource components are present.
(There will be a list of components in the box with the resource.)

4. Return the resource intact and in good condition with all the components included.

5. Acknowledge that the resource has been provided by a Research Award from the College of Nursing.

B. HOW TO RESERVE HEALTH EDUCATION RESOURCES

1. Call Ellen Stephenson, Resource Manager, at 730-7250 or email: ehs-ems@earthlink.net.  Please include:

a. Your name and title, phone number.

b. The faith community you are representing.

c. The name of the item(s) you wish to borrow. (See attached list)

 d. The dates you will need the equipment

2.  Ellen will contact you and make arrangements for the equipment pick-up and return.

3.  If your plans change, please notify Ellen as soon as possible
